Work Schedule: 100% FTE, days. Standard schedule would be between the hours of 8: 00 am - 5: 00 pm Monday through Friday. You may be eligible for up to a $3000 sign-on bonus. No on call or weekend requirements. This is a hybrid position. When in person, you will work at 705 S. University Ave in Beaver Dam, WI.

Be part of something remarkable Working as an independent healthcare provider, you'll play an integral role in our care teams by providing high-quality, cost-effective care to patients while working collaboratively within a multidisciplinary health team. This position is part of the Collaborative Care workstream in Primary Care. As a member of the Collaborative Care team, you will work as a team with the primary care staff and team psychiatrist helping deliver brief interventions for patients in Primary Care.
We are seeking a
Behavioral Health Therapist or Clinical Psychologist to:
- Evaluate patients presenting with a wide range of mental health concerns and diagnoses.
- Collaborate with patients on individualized treatment plans with specific goals and outcomes.
- Coordinate patient care with primary care, other specialty providers, and families via written and verbal communication.
- Provide psychotherapy, utilizing multiple modalities and methods consistent with evidence-based practice, including specific and focused treatment protocols, when appropriate.
- Utilize progress monitoring and outcome measurement tools to assess benefits of psychotherapy and change the treatment plan, if necessary.

If Behavioral Health Therapist: Education: Minimum - Master's Degree in Social Work, Counseling, Psychology, Marriage and Family Therapy, or other behavioral health related field
Work Experience: Minimum - 2 years of experience providing evidence-based treatment for a variety of behavioral health diagnoses for children, adolescents, or adults
Preferred - 5 years of experience providing evidence-based treatment for a variety of behavioral health diagnoses for children, adolescents, or adults
Licenses and Certifications: - Licensed Clinical Social Worker Upon Hire Required or
- Licensed Professional Counselor Upon Hire Required or
- Licensed Marriage and Family Therapist Upon Hire Required and
- Basic Life Support/CPR within 6 months Required and
- Substance Abuse Counselor Upon Hire Preferred or
- Clinical Substance Abuse Counselor Upon Hire Preferred
If Clinical Psychologist: Education: Minimum - Doctor of Philosophy (Ph.D.) or Psychology (Psy.D.) in Counseling or Clinical Psychology or related field.
Work Experience: Minimum - One (1) year of postdoctoral supervised clinical experience
Licenses and Certifications: Minimum -
- Licensure or eligible for licensure as a Psychologist in Wisconsin
- Basic Life Support/CPR
