Description:

The Case Coordinator ELEVATE is responsible for providing full range of case management services to the Five County Demonstration Project (FCDP) caseload as well as coordinating schedules with the Lead Employment Support Specialist.

RESPONSIBILITY LEVEL:

Responsible for providing full range of case management services to the Five County Demonstration Project (FCDP) caseload as well as coordinating schedules with the Lead Employment Support Specialist. Services include eligibility, enrollment, domestic violence screening, intake, referral, Employment Plan development, assessment, scheduling, follow-up, staffing, and provision of support service. Additional Responsibilities include, maintaining communication with Child Support Specialists to obtain referrals, maintaining program data base, preparing monthly program reports, completing all required state reporting forms, . Also attend weekly staffing and state conference calls. Typically works on projects and initiatives that span 3 12 months.

PRINCIPAL DUTIES:

1. Work independently, maintaining accurate and complete participant files and caseload database.

2. Provide intensive case management services ensuring communication, productivity and timeliness.

3. Act as the liaison between Child Support and Goodwill.

4. Facilitate one-on-one appointments with clients on a regular basis and providing follow up services.

5. Make the initial high-level assessment as to what specific services are needed and work with clients to develop a personalized service plan that provides for individualized assistance for the participant.

6. Introduce the program, protect the clients rights, and enroll all eligible participants.

7. Attend meetings with the state, Goodwill and Child Support.

8. Work with the Child Support Supervisor to establish clean communication between the program and other community partners providing supports to the program participants.

9. Facilitate setting up workshops and referring participants to the workshops.

10. Ensure Child Support is aware of participation of all clients at all times.

11. Enter data into WWP, KIDS, NCPATS and Excel.

12. Refer clients to workshops, job fairs, hiring events, and to other social service resources as necessary.

13. Act as a contact person for any problems and questions from the clients.

14. Create monthly reports for employment, attendance, referrals, highlights and events.

15. Work with participants in need of work supports.

16. Track attendance at appointments, identify reasons for missed appointments and assist in making new appointments.

17. Safeguard personal identifying information if there are safety and or confidentiality issues related to privacy or good cause for case participants.

18. Report on attainment of child support and employment goals as well as other agreed upon goals in the service plan.

19. Coordinate with other agencies working with mutual clients.

20. Assess participant for job readiness, education and training needs, support needs and potential exemption.

21. Maintain a safe and orderly work environment.

22. Project and Change Management: Effectively plans and manages own work to reliably complete routine job responsibilities and special assignments. May serve as a team member or subject matter expert on formal projects or within the department. Effectively engages in change affecting her/him, communicating appropriately with supervisor. Follows through on learning, skill building, and practice necessary to adapt to change.

23. Problem Solving: Light supervision; works independently. Periodic review of work by manager or senior coworker. Solves problems of moderate to advanced complexity; able to identify root cause, interpret data and resolve issues.

24. Technical Skill: Understands and applies professional principles and skills within area of specialization to modify processes to resolve situations.

25. Community Engagement: Champions Goodwill s community engagement initiatives and serves as an ambassador for Goodwill in the wider community.

26. Responsible for completing other duties/responsibilities as assigned.

REQUIREMENTS:

1. Bachelor's Degree in human services or in a related field and two years work experience in a related field.

2. Valid Driver s License and access to an insured vehicle for use in the performance of the job functions.

When determining placement within the salary range for this position Goodwill Greater Milwaukee & Chicago considers education, experience, internal equity, market demands and other qualifying criteria. It is not typical for individuals to be hired at or near the top of the rage for their role and compensation decisions are dependent on facts and circumstances of each case.
