Key Responsibilities:

• Conduct comprehensive physical therapy evaluations
• Develop and implement individualized treatment plans to promote resident mobility and function
• Monitor and adjust therapy programs based on resident progress
• Educate residents and families on therapy exercises and mobility strategies
• Maintain accurate and timely documentation of treatments and outcomes
• Collaborate with interdisciplinary teams to achieve the best outcomes for residents

Qualifications:

• Valid Physical Therapist (PT) license
• Strong communication and teamwork skills
• Dedication to enhancing residents' quality of life
