South Creek Post Acute is seeking a dedicated and knowledgeable Registered Dietitian to join our team. Position Overview
The Registered Dietitian will provide nutritional assessment, planning, and guidance for our residents, ensuring individualized, high-quality nutrition plans that support overall health and recovery. This role works closely with clinical and dietary teams to promote excellent dietary services within the facility.

Compensation
  • $45-$55 per hour, depending on experience


Key Responsibilities
  • Conduct initial and ongoing nutritional assessments
  • Develop and monitor individualized care plans
  • Work collaboratively with interdisciplinary team members
  • Provide education to residents, families, and staff
  • Ensure compliance with federal, state, and facility regulations
  • Participate in quality assurance and performance improvement initiatives


Qualifications
  • Registered Dietitian (RD) credential required
  • Current state licensure, if applicable
  • Experience in post-acute, long-term care, or a related setting preferred
  • Strong communication and organizational skills
  • Commitment to resident-centered care
