$32.00 + an hour dependable upon experience.

Bridge Crest Post Acute is a skilled nursing and rehab community. We offer short-term rehab, long-term care, and memory care, and we're a close-knit team that genuinely has each other's backs.

The role
Our social services team is going through a transition, and we need an experienced pro who can step in, get their bearings quickly, and keep things steady. You'll bring the know-how of a Director with the day-to-day focus of hands-on work: residents, families, and good discharges.

It starts as a 90-day assignment, and we're hoping it turns into something permanent. If it's a good fit on both sides, there's a real path to stay.

What your days will look like

• Getting to know residents through psychosocial assessments and care planning
• Keeping MDS and documentation accurate and on time
• Speaking up for residents and families in care conferences and IDT meetings
• Planning safe, well-coordinated discharges with hospitals, home health, and community partners
• Helping residents and families with advance directives, POLST, Medicaid, and benefits questions
• Supporting residents through behavioral health needs and the adjustment to a new setting
• Following up on concerns and grievances so people feel heard
• Lending a hand and some guidance to teammates when they need it

What you'll bring

• A degree in social work
• 3+ years in skilled nursing or post-acute social services. Director or lead experience is a big plus.
• Comfort with MDS, care plans, discharge planning, and SNF regulations
• Warmth, patience, and a knack for connecting with people on hard days
