Medical Records Assistant Status: Per Diem
Shift: Days
Pay range: $23.00 - $27.00 DOE

Apply at Teamavamere.com Essential Duties and Job Responsibilities
- Organize and maintain paper and/or electronic health records, ensuring they are complete, accurate, and easily accessible.
- Follow facility and regulatory guidelines for medical records storage, retrieval, and disposal.
- Enter and update resident information, including demographic data, diagnoses, procedures, and medications.
- Retrieve medical records for medical staff or other authorized individuals.
- Review records for completeness, accuracy, and timeliness, identifying and addressing discrepancies.
- May assist with data collection, analysis, and reporting for quality improvement or other facility purposes.
- Adhere to HIPAA regulations and maintain the confidentiality of patient information.
- Ensure the accuracy, completeness, and timely completion of medical records, including completion of discharge sheets and 360 care documentation.
- Maintain accurate records of all actions taken, including record releases, audits, and system updates.
- Effectively communicate necessary resident information to charge nurses, director of nursing, and/or administrator.
- Must be knowledgeable on federal and state laws regarding medical records.
- Review and audit admissions, discharges, change of condition, and other pertinent records on nursing units.
- Ensure that incomplete records/charts are returned to nursing service for correction.
- Ensure that registries are properly maintained for admission and discharge of residents.
- Develop and maintain a good working rapport with inter-department personnel, as well as other departments within the facility to assure that medical records can be properly maintained and problem areas can be identified and corrected.
Requirements and Qualifications
- High school diploma or equivalent.
- Basic understanding of medical terminology and anatomy/physiology.
- Proficiency in Microsoft Office Suite and/or electronic health record (EHR) systems.
- Experience in medical records or healthcare administration is preferred.
- Excellent communication and interpersonal skills.
- Strong organizational and multitasking abilities.
- The ability to work in a fast-paced, crisis-prone environment.
- Advocacy skills with a focus on patient-centered care.
