JOB OVERVIEW:

Under general supervision of a Nursing Supervisor, performs community health nursing services as a public health nurse to provide preventative medical services. Positions in this class can be distinguished by the basic nursing duties they perform in the Clinical Nursing Division.

ESSENTIAL FUNCTIONS:

Works clinical nursing programs as assigned. Including but not limited to Immunizations, Travel Immunizations, Cancer Screening, Targeted Case Management, Prenatal-5, Baby your Baby, Immigration, STD/HIV, Tuberculosis, Viral Hepatitis, Communicable disease. Provides skilled community health nursing care. Including but not limited to, assessing individual or family health needs; create nursing care plans and case management using Weber-Morgan Health Department's standing orders and following state, and federal recommendations; making appropriate referrals as necessary; providing community health education.

Provides clinical services including administering immunizations to adults and children.

Performing clinical breast exams and pap smears.

Specimen collection and preparation of specimens for laboratory testing.

Administers appropriate medication and treatments including injections and oral medication administration.

Order additional follow up and treatments ( i.e. Diagnostics, imaging, medications)

Conduct communicable disease investigations as assigned. (i.e. interviews, contact tracing, treatment, and follow-up).

Answers communicable disease questions asked by the public and other health professionals.

Carry out appropriate public health activities according to the specific program contractual requirements.

May conduct home visitation visits per program guidelines to families and individuals deemed eligible.

Evaluates and sets priorities for public health services, participates in specified programs to the community, provides health education in the community as appropriate, works with other agencies as needed to meet objectives.

Supports outreach clinical activities as assigned.

Supports, participates in the response during a crisis events, i.e. pandemic response, bioterrorism, natural disasters.

Protects client confidentiality, making sure protected health information is secured. Process departmental actions while maintaining confidentiality.

Types letters, statement, narrative and statistical reports as assigned, record minutes, agendas and other documents as required. May draft correspondence as needed.

EDUCATION/EXPERIENCE:

Education: Graduation from an accredited school of nursing with an Associate's or Bachelor's Degree.

KNOWLEDGE, SKILLS, AND ABILITIES (KSA):

Knowledge: Knowledge of community health nursing theory and practice. Working knowledge of medications, immunization schedules and ACIP guidance. Knowledge base of general communicable disease control, common communicable diseases, Universal Precautions, general public health education including nutrition, smoking cessation guidelines, exercise guidelines and disease prevention. Basic knowledge of the organization, functions, policies and procedures of the agency, some knowledge of new developments in community health nursing.

Skills: Basic clinical nursing skills including but not limited to medication administration techniques including injections, appropriate specimen collection preparation, handling and transportation, and fingerstick and venipuncture procedures. Skill in typing and using all applicable computer hardware and software applications.

Telephone system, personal computer, including work processing software and spreadsheet software (Excel, Word, Next Gen Enterprise), copy machine, cash register, fax machine, calculator, I-pads, Laptops, and radios. Medical items used in providing clinical nursing services, including but not limited to blood pressure machine (manual and automatic), stethoscope, thermometer, phlebotomy supplies, sterile equipment, injectables equipment, medications, personal protective equipment.

Working knowledge of standard office computer operations and software applications including word processing, data entry, and spreadsheets.

Abilities: Ability to apply knowledge of community health nursing theory and practice in all areas of clinical nursing services. Be able to assess patient needs, recognize contraindications, deviations from normal and Provide skilled nursing care, referral services and follow up as appropriate. Create and maintain accurate medical records. Follow written and verbal instructions, effectively communicate both verbally and in writing. Ability to establish and maintain effective working relationships with employees, other departments, and the general public. Work independently in an efficient, productive manner maintaining composure, sensitivity and confidentiality in accordance with HIPPA. Occasionally works under stressful conditions; use logic and reasoning to identify problems and solutions.

YOUR SPECIAL QUALIFICATIONS :

Must be licensed Registered Nurse in the State of Utah at time of hire.

Must possess a current, valid Utah Driver's License and must operate a motor vehicle in a safe manner.

CAREER LADDER POLICY:

Procedure: Promotional moves require the employee meet the minimum requirements of the applicable Weber County job descriptions and that the employee score 3.0 or higher on prior employee performance appraisals. Promotions will follow the Payroll Band/Grade Listing maintained by Weber County Human Resources. The employee will be raised to be the Band/Grade of the new position and their wage increased by six and a half percent (6.5%). Promotions can only be given upon completion of the requirements in the job description and will be deferred if the performance expectations are not met.

Nursing

1. Registered Nurse (RN) I: (Grade 18; $$31.34 - DOE)

a. Obtain license as Registered Nurse through Utah Department of Professional Licensing (DOPL).

b. Employee with prior related work experience may be promoted to a higher position based on evaluation of experience, i.e. RN II, or RN III.

2. RN I to RN II: (Grade 20; $34.55 - DOE)

a. Satisfy the minimum requirements of RN II job description.

b. Complete a minimum of one (1) year as an RN I or equivalent position with related experience.

c. Demonstrate proficiency and obtain the required credentials to operate at a professional level in clinical nursing services and other programs approved by the director.

3. RN II to a RN III: (Grade 22; $38.09 - DOE)

a. Satisfy the minimum requirements of RN III job description.

b. Complete a minimum of 5 years as a RN II or equivalent position with related experience.

c. Specialize at an expert level and be capable of operating in a lead role or advisory capacity for one or more clinical programs.

d. Ongoing completion of required and elective leadership trainings provided by Weber County required by management and supervisory positions but open to all staff.

e. Demonstrates leadership qualities, capable of operating in a lead role or advisory capacity for one or more clinical programs. Mentor less experienced staff and present professional image of the Weber-Morgan Health Department.

f. Completion of DIS training and Passport to Services Training

g. Complete EHR system super user e-learnings. Demonstrate proficiency in reporting, financials, and proper diagnosis and documentation

h. Maintain a positive working attitude and highest level of professionalism, conduct and productivity.

Must complete at least 2 of the following:

i. Complete a quality improvement project pertaining to the nursing division. Report to general staff and or Board of Health the plan, progress and final report.

j. Complete CIC study and Exam

k. Prepare and present a case report on a complex communicable disease investigation to nursing staff and or Epi team.

l. Prepare and present to the Board of Health on an assigned topic.
