The Licensed Master Social Worker (LMSW) provides comprehensive psychosocial support to patients and families within high-acuity neuroscience programs, including neuro-oncology and skull base. This role is responsible for assessing social, emotional, financial, and environmental factors that impact care, and for coordinating services that support continuity, access, and patient outcomes. The LMSW partners closely with a multidisciplinary clinical team to deliver patient-centered, high-touch care aligned with a concierge-level experience.

Key Responsibilities
  • Conduct comprehensive psychosocial assessments for patients and families, identifying barriers to care, social determinants of health, and support needs
  • Develop and implement individualized care plans in collaboration with providers and care teams
  • Coordinate referrals to internal and external resources, including financial assistance, behavioral health, rehabilitation, and community-based services
  • Provide brief supportive counseling, crisis intervention, and care navigation for patients with complex medical and psychosocial needs
  • Serve as a liaison between patients, caregivers, and the clinical team to ensure alignment and continuity of care
  • Support transitions of care across inpatient, outpatient, and community settings to reduce readmissions and improve outcomes
  • Educate patients and families on available resources, treatment expectations, and care planning
  • Collaborate with physicians, APPs, nursing, and administrative teams to address operational and patient care barriers
  • Document all encounters, assessments, and interventions in accordance with organizational and regulatory standards
  • Participate in program development initiatives aimed at improving patient experience and care delivery

Preferred Qualifications
  • Experience in oncology, neurology, or other specialty care settings
  • Familiarity with care navigation models and multidisciplinary team environments
  • Experience supporting uninsured or under-resourced populations

Work Environment & Requirements
  • Primarily clinic-based within the Texas Medical Center and surrounding locations
  • Occasional travel between sites may be required based on program needs
  • Valid Texas driver's license preferred; mileage reimbursement available if travel is required

Position Summary:

Independently provides professional social work services to address medical, psychiatric and social concerns by using a Biopsychosocial perspective to asses, evaluate, implement, monitor, advocate and implement services for patients and their families.

Position Key Accountabilities:
  • Conducts comprehensive assessment to interprets patients social, emotional, environmental and financial needs.
  • Plans course of treatment or refers cases to appropriate professionals, institutions or agencies to ensure that patients receive appropriate treatment or referral.
  • Obtains data from patients and/or relatives in the best interest of their problems to assist in the development of a number of significant teaching endeavors, programs, and social services.
  • With the objective of meeting total patient care, assesses patients and/or relatives to gain understanding of the individual's problems and needs. Collaborates with the physician and other members of the multi-disciplinary team to coordinate care, enhance the patient experience and achieve collaborative goals. Collaboratively develops patient goals, monitors progress, addresses barriers to goal achievement, and evaluates outcomes of individualized care plans. Conducts individual and or group counseling.
  • Prepares detailed reports and may video-tape sessions with patients for review with coworkers or a faculty member.
  • Solicits the assistance of religious, social, and welfare agencies in the treatment of patients.
  • Interacts with social work staff and psychiatric residents.
  • Provides education to physicians, nurses, and other healthcare providers on community resources and the impact of psychosocial factors on care need.
  • Utilizes crisis intervention, conflict resolution, and mediation to address patient/family concerns.
  • Responds to emergency situations.
  • Performs other duties as assigned.


Certification/Skills:
  • None
  • Manages Human Resources activities of department in regards to: recruiting and selection, hiring and termination, training, professional development, mentoring, counseling, performance evaluations, and salary planning.
  • Licensed Baccalaureate Social Worker (LBSW) by State of Texas required
  • Licensed Master Social Worker (LMSW) by State of Texas preferred

Minimum Education:
Bachelor's Degree in Social Work or related field required
Minimum Experience:
1 year of social work experience required Medical social work experience preferred
Physical Requirements:
Exerts up to 50 pounds of force occasionally and/or up to 20 pounds frequently and/or up to 10 pounds constantly to move objects.
