The Clinical Case Manager functions in a clinical role on a collaborative interdisciplinary team providing comprehensive disease management care to patients and families with complex healthcare needs. Aims to provide care that is safe, effective, patient-centered, efficient, and equitable.

Schedule: Full-Time, Mon-Fri

Position Key Accountabilities:

1. Performs patient assessment and develops a plan of care with physicians and staff in a manner that promotes quality patient care and customer service/satisfaction.
2. Plans and implements strategies to reduce the length of stay and resource consumption, and achieve positive patient outcomes.
3. Manages each patient's transition through the system and exercises judgment while coordinating care and referring patients to agencies or health care professionals for other services/transitions in care where indicated.
4. Acts as a patient and healthcare advocate by maintaining a safe environment and promoting quality healthcare and services.
5. Evaluates patients' responses and outcomes to education and therapeutic interventions.
6. Communicates effectively with collaborative team while escalating critical results and issues in an appropriate and timely manner.
7. Acts as a clinical and leadership resource for team members. Educates the staff on case management and provides specific information related to case types.
8. Respects and maintains privacy and confidentiality per HIPAA guidelines.
9. Adheres to broad scope of practice as established by the Nurse Practice Act.
10. Assures compliance with quality standards, and regulatory and accrediting agency specifications to meet federal, state and local standards.
11. All other duties as assigned.

Certification/Skills:
  • Registered Nurse (RN) State Licensure or Nurse Licensure Compact (NLC) by Texas Board of Nursing (BON) required

Minimum Education:

Four (4) years of nursing experience with a BSN from an accredited school of Nursing (preferred).
Five (5) years of nursing experience with a Nursing Diploma (three-year program).
Six (6) years of nursing experience with an ADN from an accredited school of Nursing.

Minimum Experience:

Four (4) years of nursing experience with a BSN from an accredited school of Nursing (preferred).
Five (5) years of nursing experience with a Nursing Diploma (three-year program).
Six (6) years of nursing experience with an ADN from an accredited school of Nursing.

Physical Requirements:

Exerts up to 50 pounds of force occasionally and/or up to 20 pounds frequently and/or up to 10 pounds constantly to move objects.
