Hospice Intake Coordinator

Job Summary: The primary function of the Hospice Intake Coordinator role is to facilitate the referral intake process, coordinate care with the interdisciplinary team and the patient and their family, and the referral source. A Day in the Life: Receives and appropriately documents new referrals in a timely mannerVerifies Medicare, Medicaid, and private insurance benefits timelyEnsures all physician orders, H& Ps and transfer documentation have been provided from referral source and ensures clinical documentation is reviewed by the DON and/or PCMs for clinical approvalCreates patient chart in EMR software (Consolo)Notifies clinical team of pending admissionsEnsures all physician orders, H& Ps, transfer documentation, and intake forms have been uploaded to the patients chartMaintains referral log and sends out the End of Day report dailyVerifies benefits for private insurance patients monthlyAssists in ordering equipment as directed by the DON and/or PCMsAttends weekly and monthly marketing and all staff meetingsPrepares consent and admission packetsProvides effective communication to patients/clients, staff members, other health care professionals and referral sources.Participates in the Agencys QAPI program. Qualifications: A High School Diploma, with a Bachelors Degree preferredOne year general office experience in the healthcare industry required with experience in the hospice space preferred.Computer skills using the Google SuiteExcellent interpersonal and organizational skills.Knowledge of medical terminologyValid and current auto liability insurance.
