Job Title: Licensed Social Worker

Position Summary:

We are seeking a compassionate, detail-oriented, and highly motivated Social Worker to join our team. As the only social worker in the facility, this individual will independently manage and coordinate all aspects of the social services program, ensuring residents and families receive the highest level of support. This position plays a vital role in care planning, advocacy, and compliance with state and federal regulations.

Key Responsibilities:

• Plan, develop, organize, implement, evaluate, and direct the social services programs of the facility.
• Serve as the sole social services resource for residents, families, and staff, managing all related responsibilities independently.
• Develop, administer, and maintain departmental policies and procedures in compliance with federal and state regulations and professional standards.
• Conduct annual reviews of department policies and procedures, providing recommendations for updates and improvements to the Administrator or Director.
• Stay informed of current federal and state regulations, implementing changes as needed to maintain compliance.
• Participate in interdisciplinary care planning to address residents' psychosocial needs and discharge planning.
• Provide counseling, crisis intervention, and advocacy services to residents and families.
• Collaborate with community agencies, resources, and organizations to enhance resident and family support.
• Document all social services interactions and interventions in accordance with facility standards.

Qualifications:

• Bachelor's Degree in Social Work (BSW) or related field required; MSW preferred.
• Licensed or license-eligible Social Worker in the state.
• Previous experience in a skilled nursing facility or healthcare setting strongly preferred.
• Strong knowledge of state and federal regulations regarding resident rights and social services in long-term care.
• Excellent communication, organizational, and problem-solving skills.
• Ability to work independently as the sole social worker in the facility.
