Description:
Responsibilities: - Provide daily personal care, including bathing, dressing, grooming, toileting, and hygiene.
- Assist residents with mobility, transfers, and positioning safely.
- Take and record vital signs, weights, and intake/output accurately.
- Respond promptly to call lights and ensure resident comfort and safety at all times.
- Observe and report changes in resident condition to nursing staff.
- Assist with meals, hydration, and after-meal clean-up.
- Maintain clean, organized resident rooms and handle personal items respectfully.
- Document care in accordance with Fox Hollow Post Acute policies and HIPAA guidelines.
- Support restorative and rehabilitation programs as directed by nursing.
Qualifications: - Active Certified Nursing Assistant (CNA) certification in the state.
- Strong communication, teamwork, and customer service skills.
- Ability to follow written and verbal instructions.
- Compassionate, patient, and committed to high-quality care.
Physical Requirements: - Ability to lift/move up to 25 lbs. and assist with resident transfers.
- Frequent walking, standing, bending, and hands-on care tasks.
