We are seeking a dedicated Registered Nurseto join our team and support clients in maintaining independence and quality of life at home. Position Summary The Registered Nurse provides clinical assessments, care coordination, supervision, and oversight of services for waiver program clients. This role partners closely with the Program Director and Director of Nursing to ensure quality care, regulatory compliance, and timely delivery of services. Key Responsibilities Conduct pre-screening visits and assess clients for level of care and service needsComplete admissions, required program documentation, and state forms in a timely mannerDevelop, implement, and monitor individualized service plans based on client needs and program guidelinesDocument diagnoses, medications, supplies, therapies, and service needs; coordinate follow-up to ensure timely deliverySupervise and monitor personal care attendants and delegated nursing tasks to ensure safe, quality careConduct routine client visits to evaluate service plans, client needs, and attendant performanceCoordinate with clients, families, case managers, and agency staff regarding services and plan changesEnsure compliance with waiver program requirements, infection control standards, and agency policiesComplete accurate and timely clinical documentation and required reportsParticipate in on-call rotation and provide skilled visits as needed to support client health and safetySupport continuity of care when staffing coverage is neededCollaborate with leadership to maintain quality outcomes and regulatory compliance Qualifications Education & Licensure Graduate of an accredited school of nursingCurrent Texas Registered Nurse license (or compact multistate license)IDRC Assessment training/certification required (or ability to obtain upon hire)One year of nursing experience preferred Skills & Requirements Strong clinical assessment and critical thinking skillsKnowledge of home health and waiver program regulations preferredStrong communication and documentation skillsAbility to interpret program regulations and work independentlyEnglish proficiency required; Spanish speaking is a plusValid Texas drivers license and reliable transportationClear criminal background check Physical Requirements Ability to travel independently to client homesAbility to lift and carry medical supplies and assist with patient mobility as neededAbility to perform physical tasks related to patient assessments and nursing care, including transfers and use of adaptive equipment Why Join Caprock Home Health?
