Description:

Position: IT Clinical Informatics Analyst (HCHB SME)
We're seeking an experienced IT Clinical Informatics Analyst to serve as a Subject Matter Expert (SME) for Homecare Homebase (HCHB). This role supports and optimizes the platform across Home Health and Hospice, improving workflows, system performance, and data integrity.
Key Responsibilities
  • Act as SME for HCHB across Home Health and Hospice workflows
  • Troubleshoot system issues and support configuration improvements
  • Analyze and optimize clinical workflows for efficiency and standardization
  • Collaborate with vendor on system updates and enhancements
  • Support reporting, data validation, and accuracy
  • Participate in system projects and informatics initiatives
  • Ensure HIPAA compliance and maintain documentation
Qualifications
  • Bachelor's degree or equivalent experience
  • Strong hands-on HCHB experience required
  • Experience in Home Health and/or Hospice
  • Knowledge of OASIS, PDGM, and Hospice workflows
  • Ability to optimize EHR systems beyond implementation
  • Strong analytical and communication skills
