Job Type: Full-time

Lebanon Center for Rehabilitation and Healing is seeking an organized and detail-oriented Licensed Practical Nurse (LPN) to serve as our Medical Records LPN.

The Medical Records LPN is responsible for maintaining complete, accurate, and confidential resident health records in accordance with facility policies and applicable regulations. The ideal candidate combines clinical knowledge with strong administrative, organizational, and communication skills.

Responsibilities

  • Maintain accurate, complete, and organized resident medical records

  • Review charts for missing, incomplete, or inconsistent documentation

  • Conduct routine chart audits and follow up with appropriate team members

  • File and process physician orders, laboratory reports, consultation notes, and other clinical documentation

  • Ensure records are updated promptly following admissions, transfers, and discharges

  • Track required documentation, signatures, assessments, and care-plan materials

  • Process authorized requests for medical records in accordance with HIPAA and facility policies

  • Safeguard the privacy and confidentiality of resident information

  • Assist nursing and interdisciplinary team members with medical-record documentation needs

  • Prepare records for audits, surveys, reviews, and regulatory requests

  • Maintain archived records according to established retention requirements

  • Report documentation concerns or deficiencies to nursing leadership

  • Perform other nursing and medical-record duties as assigned

Qualifications

  • Current, unrestricted LPN license in the state of employment

  • Previous experience in skilled nursing, rehabilitation, long-term care, or another healthcare setting preferred

  • Medical records or health information experience strongly preferred

  • Knowledge of HIPAA, confidentiality standards, and clinical documentation practices

  • Familiarity with electronic medical record systems preferred

  • Strong organizational and time-management skills

  • Excellent attention to detail and accuracy

  • Effective written and verbal communication skills

  • Ability to manage multiple priorities and meet deadlines

  • Ability to work collaboratively with nursing staff, physicians, residents, families, and facility leadership
