Position Summary

We are seeking a skilled and compassionate Occupational Therapist (OTR) to provide patient-centered therapy services on a PRN basis. In this role, you'll help residents improve function, independence, and quality of life while collaborating with an interdisciplinary care team.
Key Responsibilities
  • Evaluate residents and develop individualized treatment plans
  • Provide therapy services per physician orders and care goals
  • Document evaluations, treatments, and progress notes accurately and timely
  • Collaborate with physicians, nursing, and therapy team members
  • Educate residents, families, and caregivers on therapy programs
  • Participate in care planning and discharge planning
  • Conduct in-services and staff training as needed
  • Ensure compliance with all state, federal, and payer guidelines
Qualifications
  • Degree in Occupational Therapy (Bachelor's required; Master's/Doctorate preferred)
  • Active South Carolina OT license (OTR) in good standing
  • Experience in skilled nursing or rehab setting preferred
  • Familiarity with PointClickCare (PCC) and Casamba preferred
  • Strong documentation, communication, and clinical skills
