Position Summary

We are seeking a compassionate and skilled Occupational Therapist (OTR) to provide patient-centered therapy services that improve residents' independence and quality of life. This role focuses on evaluation, treatment planning, and collaboration with the interdisciplinary care team.
Key Responsibilities
  • Evaluate residents and develop individualized treatment plans
  • Provide therapy services per physician orders and care goals
  • Document evaluations, treatments, and progress notes accurately and timely
  • Communicate with physicians, nursing staff, and interdisciplinary team members
  • Educate residents, families, and caregivers on therapy programs
  • Participate in care planning and discharge planning
  • Conduct staff in-services and training
  • Maintain compliance with all state, federal, and payer regulations
Qualifications
  • Degree in Occupational Therapy (Bachelor's required; Master's or Doctorate preferred)
  • Active South Carolina Occupational Therapist (OTR) license (in good standing)
  • Experience in skilled nursing or rehab setting preferred
  • Familiarity with PointClickCare (PCC) and Casamba preferred
  • Strong clinical, documentation, and communication skills
