We are looking for a dynamic
MDS Nurse (RN) who is passionate about team success and easily builds and maintains positive relationships with peers. The ideal candidate will have a strong background in
LTC Skilled Nursing and has demonstrated leadership abilities. This position requires excellent communication skills, attention to detail, and the ability to work well under pressure.

Successful MDS Nurse candidates will have the following: - Nursing Degree from an accredited College/University
- Must possess a current, unencumbered license to practice as a Registered Nurse (RN) or Licensed Practical Nurse (LPN) in South Carolina
- A minimum of 3 to 5 years progressive supervisory/managerial experience in SNF is preferred
- Current CPR certification
- Experience with PCC preferred
- Must maintain all required continuing education/ licensing and remain in good standing with the State Board of Nursing.
Primary responsibilities include but not limited to: Ensures the accurate and timely completion of all MDS Assessments including PDPM Medicare, quarterly, annual, significant change. Communicates level of care for new resident to all disciplines. Coordinates interdisciplinary participation in completing the Minimum Data Set (MDS) for each new admission to facility according to regulatory time frames. Ensures completeness and thoroughness of documentation as mandated by federal, state and medical standards. Maintains an accurate schedule of all MDS assessments to include the proper reference dates throughout the resident's stay.
