Position Summary

We are seeking an experienced MDS Coordinator (RN) to ensuring accurate and timely completion of MDS assessments in compliance with state, federal, and Medicare guidelines.

Key Responsibilities
  • Complete and submit MDS assessments accurately and on time
  • Ensure compliance with CMS, Medicare, and Medicaid regulations
  • Coordinate with the interdisciplinary team for care plan development
  • Review and monitor documentation to support reimbursement accuracy (PDPM)
  • Participate in care plan meetings and QA processes
  • Track and manage assessment schedules and deadlines
  • Educate staff on documentation and regulatory requirements
  • Collaborate with nursing, therapy, and administration teams

Qualifications
  • Active RN license in South Carolina (in good standing)
  • Prior MDS experience required
  • Strong knowledge of PDPM, Medicare/Medicaid, and long-term care regulations
  • Experience in skilled nursing facility (SNF) setting preferred
  • Excellent attention to detail and organizational skills
  • Strong communication and teamwork abilities
