Position SummaryWe are seeking an experienced
MDS Coordinator (RN) to ensuring accurate and timely completion of MDS assessments in compliance with
state, federal, and Medicare guidelines.
Key Responsibilities- Complete and submit MDS assessments accurately and on time
- Ensure compliance with CMS, Medicare, and Medicaid regulations
- Coordinate with the interdisciplinary team for care plan development
- Review and monitor documentation to support reimbursement accuracy (PDPM)
- Participate in care plan meetings and QA processes
- Track and manage assessment schedules and deadlines
- Educate staff on documentation and regulatory requirements
- Collaborate with nursing, therapy, and administration teams
Qualifications- Active RN license in South Carolina (in good standing)
- Prior MDS experience required
- Strong knowledge of PDPM, Medicare/Medicaid, and long-term care regulations
- Experience in skilled nursing facility (SNF) setting preferred
- Excellent attention to detail and organizational skills
- Strong communication and teamwork abilities
