Position Summary
We are seeking compassionate and dedicated Licensed Practical Nurses (LPNs) to join our nursing team. The LPN is responsible for providing direct nursing care, administering medications and treatments, monitoring resident conditions, and collaborating with the interdisciplinary team to ensure exceptional resident outcomes.
Key Responsibilities
- Provide direct nursing care in accordance with physician orders and individualized care plans
- Administer medications and treatments safely and accurately
- Monitor residents' conditions and promptly report changes
- Maintain accurate and timely clinical documentation
- Supervise and support Certified Nursing Assistants (CNAs)
- Communicate effectively with residents, families, physicians, and interdisciplinary team members
- Respond appropriately to resident needs, emergencies, and changes in condition
- Promote resident dignity, comfort, and quality of life
- Ensure compliance with all state, federal, and facility regulations
- Utilize PointClickCare (PCC) and other clinical systems as needed
Qualifications
- Current, unrestricted LPN license in South Carolina
- Current CPR certification
- Must maintain all required continuing education and licensure requirements
- Experience in skilled nursing or long-term care preferred
- Experience with PointClickCare (PCC) preferred
- Strong clinical assessment, communication, and organizational skills
- Compassionate and resident-focused

- Strong teamwork and leadership skills
- Excellent communication and documentation abilities
