Position Summary
We are seeking a licensed Occupational Therapist (OT) to provide patient-centered therapy services focused on improving residents' independence and quality of life. This role involves evaluation, treatment planning, interdisciplinary collaboration, and documentation compliance.
Key Responsibilities
- Evaluate residents and develop individualized treatment plans
- Provide skilled occupational therapy services per physician orders
- Document evaluations, treatments, and progress notes in compliance with regulations
- Collaborate with physicians, nursing, and interdisciplinary team members
- Educate residents, families, and caregivers on treatment plans and home programs
- Participate in care planning, discharge planning, and team meetings
- Conduct staff training and in-service education
- Maintain compliance with state practice acts and facility policies
- Degree in Occupational Therapy (Bachelor's required; Master's or Doctorate preferred)
- Active Occupational Therapist license in South Carolina (in good standing)
- Experience in skilled nursing or rehabilitation setting preferred
- Familiarity with PointClickCare and Casamba preferred
- Strong clinical, documentation, and communication skills
