Position SummaryAs a
Physical Therapist (PT), you will evaluate and treat residents to improve mobility, strength, and functional independence. You will collaborate closely with physicians, families, and interdisciplinary team members to provide personalized, high-quality care.
Key Responsibilities- Perform patient evaluations and develop individualized treatment plans
- Provide therapy to improve mobility, balance, strength, and function
- Monitor patient progress and adjust treatment plans as needed
- Maintain accurate and timely clinical documentation
- Communicate with physicians, residents, families, and care team members
- Participate in care planning and discharge planning
- Conduct staff education and in-service training
- Supervise PTAs, aides, and students
Qualifications- Degree in Physical Therapy (Bachelor's required; Master's or Doctorate preferred)
- Active, unencumbered Physical Therapist license in South Carolina (required)
- Must maintain all continuing education and remain in good standing with the State Board
- Experience in a skilled nursing or rehab setting preferred
- Familiarity with PointClickCare (PCC) and Casamba preferred
- Strong clinical, communication, and leadership skills
