Sr. Medical Director Value Based Care-Southwestern Health Resources-Clinically Integrated Network (SWHR-CIN)

Position Summary

Provides physician leadership for value-based care strategies across the Clinically Integrated Network (CIN), including oversight of quality, cost, utilization, and patient outcomes. Partners with physicians, care management, operations, finance, and analytics teams to drive performance improvement, standardize care delivery, and support population health initiatives. Serves as a key clinical leader to advance value-based care models and organizational goals.

Position Duties

Leads clinical strategy and performance for value-based care programs and
contracts, including quality, cost, and utilization outcomes

Partners with physicians and provider leaders to promote alignment, engagement, and accountability for performance results. Support development of program education and partner with providers to ensure understanding and effective implementation

Uses data and analytics to identify gaps in care, variation, and opportunities for
improvement

Collaborates with care management and population health teams to support
chronic disease management and preventive care initiatives

Establishes and promotes evidence-based standards, care pathways, and best
practices across the network

Ensures compliance with quality, regulatory, and accreditation requirements;
supports performance improvement initiatives

Works with executive leadership on strategic initiatives, payer partnerships, and
value-based contracting from a clinical perspective

Serves as physician leader in committees, councils, and multidisciplinary teams

Education
M.D. or D.O. Active, unrestricted medical license required; Board certification required

Experience
7-10 years of clinical practice experience required
Leadership experience in value-based care, population health, or physician organizations required
Experience with clinically integrated networks (CIN) or physician alignment models required
Experience with risk-based or shared savings programs preferred

Licenses and Certifications
MD - Medical Doctor, or, DO - Doctor of Osteopathic Medicine required upon hire
Board certification - specialty board certification required upon hire

Skills

. Data-driven decision-making and performance management capabilities

. Effective communication, collaboration, and relationship-building skills

. Knowledge of quality metrics, population health, and care delivery models

Travel Requirements
Local 10% Meetings with physicians, clinics, and system leadership
