Hours: 40 hours/week - must be flexible; some On-Call

Overview:

We are seeking a Residential Family Living Program Specialist to join our team in delivering compassionate, person-centered care through our Lifesharing model. A typical day-to-day may include:

  • Coordinating and conducting monthly visits to Lifesharing homes to support individuals and providers
  • Developing and updating Individual Support Plans (ISPs) in collaboration with stakeholders
  • Delivering training to Lifesharing providers on health, safety, and program expectations
  • Monitoring service delivery and ensuring compliance with state and county regulations
  • Supporting individuals with budgeting, benefits management, and financial education
  • Participating in intake, admission, discharge, and annual meetings to ensure continuity of care
  • Conducting assessments and health screenings, and tracking fire drills and self-preservation activities
  • Maintaining accurate documentation in CareLogic and overseeing individual spending and reporting

What you’ll have:

  • A master’s degree or above from an accredited college or university and 1 year of work experience working directly with individuals with an intellectual disability or autism.
  • A bachelor’s degree from an accredited college or university and 2 years’ experience working directly with individuals with an intellectual disability or autism.
  • An associate’s degree or 60 credit hours from an accredited college or university and 4 years of experience working directly with individuals with an intellectual disability or autism
  • Valid driver’s license
  • Strong understanding of regulatory standards and documentation practices
  • Ability to work effectively with diverse stakeholders and manage multiple responsibilities
