*Relocation Assistant Available* Join a collaborative, therapist-led team at Avamere Three Fountains, a 117-bed skilled nursing facility with a spacious therapy gym, kitchen, and access to a health club and swimming pool for therapy sessions. We offer in-house therapy services across Avamere communities in Oregon and Washington-no travel between buildings required.

Your Role: - Carry out treatment plans under supervision of a Physical Therapist
- Document care per facility and insurance requirements
- Communicate with patients, families, and interdisciplinary teams
- Report changes in patient status
- Participate in department operations and development
What You Need: - Graduate of an accredited PTA program
- Unencumbered license and in good standing with licensing boards
- Knowledge of Medicare and payer regulations
- CPR certification
- Professionalism, strong communication, and multitasking skills
