The Supervisor, Integrated Care Teams role is a unique leadership position that supports OHTs’ strategy to improve client transitions through Hospital-to-Home and other integrated care programs like the Leading Practice (LP) projects that involve linking hospital and community partners to support earlier discharges of adults with higher needs to reintegrate into their home and community, within an Integrated Care model.
The Supervisor, Integrated Care Teams blends clinical expertise with operational oversight, ensures coordinated scheduling, coaches an integrated care approach within the team and across the partners, and ensures ongoing communication with primary care, Ontario Health atHome (OHAH) and other community services. The Supervisor, Integrated Care Teams works within a client and family-centered philosophy to provide clinical leadership, expertise in home care, and cultivates a collaborative, solutions-oriented environment that will advance the strengths and resources across the partners to address client and family needs. This leadership position will provide direction and operational oversight to both regulated and unregulated health professionals and may have shared supervisory accountability for rehab, nursing, personal support and homemaking roles. A priority is an autonomous and self-directed practice that focuses on relationship building and engagement across sectors.

Key Areas of Accountability

  • Provides supervision, clinical leadership, advice, consultation, mentorship to an interprofessional and integrated home healthcare team across different funding models (OHAH and other funders).
  • Facilitates team development through recruitment, orientation, ongoing training, performance evaluations, and professional development of team members.
  • Provides practice oversight to the assigned members of the Integrated Care Team and ensures regular communication with partner organizations and funders
  • Fosters effective interdisciplinary collaboration to optimize care delivery, enhance client outcomes and promote client safety and risk reduction
  • Works within, and follows the policies, procedures and regulations related to staff safety as required by VHA and under the Occupational Health and Safety Act
  • Completes comprehensive client assessment, and provides direct care in the home as needed to support complex client cases or evaluate clinical practices, outcomes achievement and client support and safety
  • Acts as a key change agent in supporting organizational change, quality improvement and quality assurance by monitoring key performance indicators (KPIs), resource utilization, and clinical outcomes
  • Functions as a member of the Regional and Client Services teams and contributes to team and service effectiveness through participation in supervisory coverage through rotational on-call and after-hours support

Qualifications

  • Must be a Registered Nurse or Registered Practical Nurse in good standing with the College of Nurses of Ontario.
  • Good understanding of the health care services and delivery processes in the community and home care sector.
  • Experience working within an interprofessional or integrated care approach is essential.
  • Knowledge and experience with gerontology
  • Intersectoral experience and excellent relationship building skills and dispute resolution
  • Demonstrated commitment to client and family centered care.
  • Strong clinical practitioner with an ability to use knowledge-transfer techniques to put evidence-based knowledge into practice.
  • Excellent leadership and facilitation skills and the ability to coach and motivate teams.
  • Demonstrated knowledge and/or proficient skills in the use of information technology for electronic clinical documentation and clinical decision support.
  • Knowledge and experience in the use of standardized assessment tools in care plan development an asset.
  • Awareness and understanding of all relevant legislation related to practice standards, standards of safety and quality, and employment standards including human rights legislation.
  • Demonstrated leadership skills with strong organizational and self-management skills, and with demonstrated ability to achieve personal, professional, and organizational performance targets.
  • Superior interpersonal, verbal and written communication skills, and skilled in customer service.
  • Clear, logical, system thinker and innovative problem solver with demonstrated advanced judgement and decision-making skills.
  • Demonstrated ability to work independently, and to lead or contribute as a member of an interprofessional team.
  • Demonstrated commitment to modelling lifelong learning and continuous professional growth.
  • Use of proper body mechanics and successful completion of the Physical Readiness Test to ensure safe work practices are followed, including mobility, transfers, bending, lifting, pushing and pulling.
  • Valid driver’s license and use of a car on the job required.
