Description:

Assists with the daily functions, patient outreach, and identification of community resources for patients, patient counseling and education for the multi-disciplinary clinic.

Duties:

  • Responsible for patient counseling and referrals for patient needs.
  • Assists with coordinating patient care.
  • Responsible for the on-going daily management of patient continuity of care for patients entering and going through the multi-disciplinary clinics.
  • Organizes and assists with conducting community outreach on behalf of the Program.
  • Maintains a customer service program to ensure a high quality service and customer satisfaction for patients in the clinical setting.
  • Creates and conducts surveys for evaluating satisfaction of the users of the program.
  • Participates in and initiates advancement of hospital, community and patient relations.
  • Survey and follow-sup with referring providers to ensure high quality customer service.
  • Meets with patients to ensure customer satisfaction with the Program and hospital experience.
  • Utilizes and/or creates databases for tracking and following patients through out-patient clinical settings.
  • Organizes and analyzes program data on a regular basis.
  • Creates and maintains referring physician informational databases.
  • Maintains all documentation and files on behalf of the program.
  • Performs various duties as needed to successfully fulfill the function of the position.

Educational requirements:

  • high school

Desired Skills:

Required Education and Experience: Bachelor's Degree in Health Education, Social Work, Psychology, Nursing, or health related field AND:

  • 12 months experience in a medical or clinical care setting.

Equivalency/Substitution: Will accept 48 months related experience in lieu of the Bachelor's Degree for a total of 60 months experience.

Certifications:

  • A license from the State of Oklahoma related to the required health field.
