Mays Housecall Home Health of Durant is hiring a Hospice On-Call RN – After Hours in Durant, OK. This is a full-time position.
Hospice On-Call RN After Hours We are seeking an experienced Hospice Registered Nurse (RN) to provide on-call nursing support to hospice patients and families during after-hours, evenings, nights, and weekends. This position is responsible for responding to patient and caregiver needs outside of regular business hours, including telephone triage, symptom management, urgent visits, and coordination of care. The ideal candidate is an experienced hospice nurse who is comfortable working independently and making sound clinical decisions in an on-call environment. What Youll Do Provide after-hours on-call nursing support for hospice patients and familiesRespond promptly to patient and caregiver calls regarding changes in condition, symptoms, medications, or other clinical concernsProvide telephone triage and determine when an in-person nursing visit is neededMake after-hours patient visits as clinically appropriateAssess and manage pain, symptoms, and changes in patient conditionProvide education, guidance, and emotional support to patients and caregiversCoordinate with physicians, pharmacies, facilities, and other members of the hospice team as neededCommunicate significant changes in patient condition to the appropriate clinical team membersComplete timely and accurate documentation of calls, visits, assessments, and interventionsSupport patients and families during end-of-life situationsEnsure care is provided in accordance with hospice regulations, company policies, and clinical standards What Were Looking For Active Registered Nurse (RN) license requiredHospice nursing experience requiredStrong hospice clinical knowledge, including pain and symptom managementPrevious on-call, after-hours, or triage experience preferredStrong clinical judgment and ability to make independent decisionsExcellent communication and organizational skillsAbility to respond appropriately to urgent patient and caregiver needsComfortable traveling to patient homes and facilities during assigned on-call hoursValid drivers license and reliable transportationCurrent CPR certification
