Description:

What You'll Do
  • Communicate regularly with residents and families regarding plans of care, therapy progress, treatment protocols, and resident status updates
  • Participate in daily stand-up and interdisciplinary meetings to support seamless communication across departments
  • Answer resident and family questions regarding care plans, illness progression, treatments, and services to promote comfort and confidence
  • Monitor and assess resident conditions, adjusting care needs as appropriate
  • Prioritize and organize daily clinical responsibilities in a high-volume environment
  • Complete accurate and timely documentation and charting
  • Perform utilization review activities to ensure appropriate, timely, and cost-effective resident care
  • Coordinate care with physicians, ancillary providers, financial services, and third-party payers
  • Oversee admissions, transfers, and discharge processes
  • Coordinate referrals to and from the facility
  • Promote teamwork, collaboration, and professional communication among staff and healthcare partners
  • Exercise independent judgment while maintaining professionalism and compassion in all interactions
What We're Looking For
  • Active RN license in good standing
  • Minimum of 2 years of clinical nursing experience preferred
  • Knowledge of Medicare, Medicaid, and managed care processes preferred
  • PCC experience a plus
  • Excellent communication, organization, and time management skills
  • Ability to work independently and adapt to changing priorities
  • Strong documentation and computer skills
  • Commitment to respectful, resident-centered care

Pay Rate $85, 000-$95, 000
