Key Responsibilities:

• Evaluate and treat residents in accordance with physician orders and individualized therapy plans
• Develop comprehensive treatment goals and monitor resident progress
• Educate residents, families, and caregivers to support functional gains
• Collaborate with an interdisciplinary team to ensure the best clinical outcomes
• Maintain accurate and timely documentation in accordance with regulatory standards

Qualifications:

• Licensed Physical Therapist (PT) in the state of [Insert State]
• Degree from an accredited Physical Therapy program
• CPR certification (or ability to obtain)
• Previous experience in a skilled nursing or long-term care setting preferred
