RN MDS Nurse Coordinator
Dublin Post Acute Care

Dublin Post Acute is seeking a dedicated and detail-oriented RN MDS Nurse Coordinator to join our team. The MDS Nurse plays a key role in ensuring accurate and timely completion of resident assessments, coordinating care planning, and supporting compliance with state and federal regulations.

Responsibilities:

• Complete and manage MDS assessments in compliance with regulatory guidelines.
• Coordinate and oversee interdisciplinary care planning.
• Ensure accuracy of documentation and coding for reimbursement.
• Collaborate with nursing, therapy, and other care team members.
• Monitor and maintain compliance with quality measures and facility policies.

Qualifications:

• Current RN license in the state of Ohio.
• Prior MDS/RAI experience preferred but willing to train the right candidate.
• Strong organizational and assessment skills.
• Knowledge of long-term care regulations and requirements.
• Ability to work independently and as part of a team.
