Job Title: Social Service Designee
Job Summary

The Social Service Designee is responsible for assisting residents/patients and their families with social, emotional, and discharge planning needs. This role supports overall well-being by coordinating services, advocating for residents, and ensuring compliance with applicable regulations.
Key Responsibilities
  • Assess residents' social, emotional, and psychological needs upon admission and throughout their stay
  • Develop and implement individualized care plans in collaboration with the interdisciplinary team
  • Provide counseling and emotional support to residents and families
  • Assist with discharge planning, including coordinating community resources and services
  • Maintain accurate and timely documentation in accordance with state and federal regulations
  • Advocate for residents' rights and ensure dignity and respect are upheld
  • Facilitate communication between residents, families, and healthcare staff
  • Coordinate social services programs such as support groups or recreational activities
  • Assist with resolving conflicts or concerns involving residents and families
Qualifications
  • High school diploma or equivalent required; associate or bachelor's degree in social work, human services, or related field preferred
  • Previous experience in healthcare, long-term care, or social services preferred
  • Knowledge of basic social service practices and community resources
  • Strong interpersonal, communication, and organizational skills
  • Ability to handle sensitive information with confidentiality
  • Compassionate and patient-centered approach
Work Environment
  • Typically works in nursing homes, assisted living facilities, rehabilitation centers, or hospitals
  • May involve interaction with elderly, disabled, or chronically ill individuals
  • Combination of desk work and direct resident interaction
