ob Title: Full-Time Social Services Coordinator
Location: Centerville Post Acute - Centerville, Ohio
Position Summary: Centerville Post Acute is seeking a compassionate, empathetic, and hardworking Social Services Coordinator with a valid social service license to join our interdisciplinary care team. This full-time role is responsible for supporting residents' psychosocial well-being, assisting with care planning, and ensuring compliance with state and federal regulations. The ideal candidate is dedicated, reliable, and passionate about improving the quality of life for individuals in a post-acute care setting.
Key Responsibilities: - Conduct comprehensive psychosocial assessments for residents upon admission and as needed
- Develop, implement, and update individualized care plans in collaboration with the care team
- Provide counseling and emotional support to residents and their families with compassion and empathy
- Assist with discharge planning, including coordination of community resources and services
- Advocate for residents' rights and ensure dignity and respect in all interactions
- Maintain accurate and timely documentation in accordance with facility policies and regulatory requirements
- Participate in care conferences and interdisciplinary team meetings
- Address concerns related to abuse, neglect, or psychosocial distress
- Facilitate communication between residents, families, and healthcare providers
Qualifications: - Active Social Services license required (LSW, LISW, or equivalent per Ohio regulations)
- Bachelor's degree in Social Work or related field (Master's preferred)
- Minimum of 1-2 years of experience in healthcare, long-term care, or post-acute setting preferred
- Demonstrated compassion, empathy, and a strong work ethic
- Strong communication, organizational, and interpersonal skills
- Ability to handle sensitive situations with professionalism and care
- Knowledge of state and federal regulations related to long-term care

Schedule: - Full-time position
