Position Summary:

Buckeye Care and Rehabilitation is seeking a dedicated, compassionate, and reliable Licensed Practical Nurse (LPN) to join our skilled nursing team. The LPN will play a vital role in delivering high-quality, person-centered care to our residents while working collaboratively with our interdisciplinary team. Key Responsibilities:
  • Provide direct nursing care to residents in accordance with their care plans.
  • Administer medications and treatments as prescribed by the attending physician.
  • Monitor residents' conditions and report significant changes to the RN supervisor or physician.
  • Accurately document all nursing care provided.
  • Collaborate with nursing assistants and other healthcare staff to ensure a safe, clean, and caring environment.
  • Participate in care conferences and family communication as needed.
  • Ensure compliance with state, federal, and facility regulations and policies.
  • Uphold resident rights, dignity, and confidentiality.
Qualifications:
  • Current and valid LPN license in the state of Ohio (or eligibility for licensure).
  • Graduate of an accredited practical nursing program.
  • Previous experience in a skilled nursing facility or long-term care setting preferred.
  • Strong clinical, communication, and organizational skills.
  • Ability to work effectively as part of a multidisciplinary team.
  • CPR/BLS certification
Work Schedule:
  • Full-Time
  • Must be available to work rotating weekends, holidays, and various shifts as needed (Days/Evenings/Nights).
