Position Summary: Buckeye Care and Rehabilitation is seeking a dedicated, compassionate, and reliable
Licensed Practical Nurse (LPN) to join our skilled nursing team. The LPN will play a vital role in delivering high-quality, person-centered care to our residents while working collaboratively with our interdisciplinary team. This is an excellent opportunity for an LPN who thrives in a fast-paced environment and is committed to enhancing the lives of seniors through excellent clinical care.
Key Responsibilities: - Provide direct nursing care to residents in accordance with their care plans.
- Administer medications and treatments as prescribed by the attending physician.
- Monitor residents' conditions and report significant changes to the RN supervisor or physician.
- Accurately document all nursing care provided.
- Collaborate with nursing assistants and other healthcare staff to ensure a safe, clean, and caring environment.
- Participate in care conferences and family communication as needed.
- Ensure compliance with state, federal, and facility regulations and policies.
- Uphold resident rights, dignity, and confidentiality.
Qualifications: - Current and valid LPN license in the state of Ohio (or eligibility for licensure).
- Graduate of an accredited practical nursing program.
- Previous experience in a skilled nursing facility or long-term care setting preferred.
- Strong clinical, communication, and organizational skills.
- Ability to work effectively as part of a multidisciplinary team.
- CPR/BLS certification
Work Schedule: - Full-Time
- Must be available to work rotating weekends, holidays, and various shifts as needed (Days/Evenings/Nights).
