The Chateau at Brooklyn Rehabilitation and Nursing Center is seeking a compassionate and experienced Wound Care LPN to join our exceptional nursing team. This is an exciting opportunity for a dedicated nurse who is passionate about wound management and providing outstanding care to our residents.

Position Summary

The Wound Care LPN is responsible for providing specialized wound care services, assisting with the prevention and treatment of pressure injuries, and ensuring optimal healing outcomes for residents in our skilled nursing facility.

Responsibilities
  • Perform wound assessments and provide treatment in accordance with physician orders and facility protocols.
  • Monitor and document wound progress and communicate changes to the interdisciplinary team.
  • Assist in developing and implementing individualized wound care plans.
  • Ensure accurate and timely documentation of all treatments and assessments.
  • Educate residents, families, and staff on wound prevention and treatment strategies.
  • Participate in skin rounds and quality improvement initiatives.
  • Maintain compliance with all federal, state, and facility regulations and standards.
  • Collaborate with physicians, nurse practitioners, and nursing staff to ensure excellent resident outcomes.
Qualifications
  • Current and active New York Licensed Practical Nurse (LPN) license.
  • Previous wound care experience in a skilled nursing facility or long-term care setting preferred.
  • Wound Care Certification is a plus.
  • Strong clinical assessment and documentation skills.
  • Excellent communication and organizational abilities.
  • Passion for delivering exceptional resident-centered care.
