Liberty Resources’ Integrated Health Center in Fulton, NY is currently seeking a Licensed Practical Nurse (Primary Care) to support our growth and expansion.

Licensed Practical Nurse (LPN) Position Summary:
The Licensed Practical Nurse (LPN) functions as an integral member of the Primary Care Team, providing clinical support and direct patient care under the supervision of a registered nurse and/or primary care provider. The LPN assists in delivering comprehensive healthcare services, promotes preventive care and wellness, supports chronic disease management, and helps ensure efficient clinic operations.
License Practical Nurse (LPN) Job Responsibilities:
- Assist providers with patient examinations, procedures, and treatments.
- Perform patient intake, including obtaining vital signs, medical histories, medication reconciliation, and documenting patient concerns.
- Provide health screenings and routine clinical assessments within scope of practice.
- Administer medications, immunizations, and treatments as ordered by the provider and in accordance with agency policies.
- Perform phlebotomy and specimen collection procedures for which the LPN is trained.
- Provide patient education regarding preventive health, chronic disease management, medications, treatment plans, and healthy lifestyle choices.
- Support care coordination by communicating with patients, families, pharmacies, specialists, and community resources as appropriate.
- Assist with medication refills, prior authorizations, and referrals to specialty care providers.
- Review laboratory and diagnostic results with providers and assist with patient follow-up as directed.
- Monitor patients with chronic health conditions and facilitate follow-up care and preventive screenings.
- Maintain accurate and timely documentation within the electronic medical record (EMR).
- Ensure compliance with clinical quality measures, infection control standards, and agency policies.
- Respond appropriately to urgent medical situations and assist with emergency interventions as needed.
- Participate in team meetings, quality improvement initiatives, and other clinic activities.
- Perform other duties, responsibilities, and related tasks as assigned.
Licensed Practical Nurse (LPN) Qualifications:
- Graduate of an accredited Licensed Practical Nursing (LPN) program.
- Current New York State LPN license in good standing.
- Minimum of one year of clinical nursing experience preferred; experience in primary care, family medicine, ambulatory care, or community health strongly preferred.
- Eligibility for full and unconditional participation in Medicare and Medicaid programs.
- Strong assessment, organizational, and patient education skills.
- Excellent verbal and written communication skills.
- Ability to work effectively within a multidisciplinary healthcare team.
- Proficiency in utilizing electronic medical records (EMR) systems.
- Current BLS certification required.
- Valid New York State driver's license and reliable transportation when required.
