Community Based Therapist

Liberty Resources is currently seeking a Community Based Therapist for our Integrated Health Care clinic in Fulton, NY.

Position Summary:

Under the direct supervision of the Supervising Therapist, the Community Therapist is primarily responsible for the provision of therapy toindividuals, couples, families and groupsreferred to the Integrated Health Care Clinic.

Essential Job Functions:

• Provides person-centered, recovery-focused individual, family, couples, and group psychotherapy using evidence-based practices to support identified treatment goals.
• Conducts intake assessments, clinical evaluations, case conceptualization, and individualized treatment planning in collaboration with clients, families, and clinical supervisors.
• Provides primarily community-based services to individuals and families who have difficulty engaging in traditional clinic-based care.
• Delivers crisis intervention, advocacy, care coordination, and linkages to educational, legal, healthcare, and community resources as needed.
• Maintains effective collaboration with clients, families, service providers, community organizations, and interdisciplinary team members to support positive outcomes.
• Completes timely and accurate clinical documentation, treatment plans, reports, statistics, case records, incident reporting, and other required documentation in accordance with agency, OMH/AAPC, and program standards.
• Participates in supervision, staff meetings, trainings, and professional development; maintains required licensure/certifications, provides one evening of clinical services per week until 7: 00 p.m., and consistently meets the expectation of 21 hours of direct clinical services per week after the 10-week productivity build-up period, unless otherwise authorized.

Salary Range: $66, 000

Qualifications:

• Master’s degree in Social Work, Mental Health Counseling, or Marriage and Family Therapy, with one (1) year of post-graduate experience; related experience may substitute.
• Current NYS license or permit to practice in the applicable field.
• Demonstrated clinical knowledge and skills to support clients in achieving identified treatment goals.
• Flexible, family-oriented approach with the ability to work one evening per week.
• Excellent verbal and written communication skills.
• Proficiency with electronic medical records and timely, accurate clinical documentation.
