JOB SUMMARY
Plan, coordinate, and direct a vibrant community life program and meaningful life enrichment activities, based on residents’ interests, preferences, abilities and needs. Recruit, train, supervise, and support life enrichment assistants and volunteers. Develops internal and external community-life partnerships and resources. Coordinates the life enrichment resident review and contributed to resident care plans, and other required documentation.

POSITION OVERVIEW
· Organize programs for group and individual activities based on each resident’s individualized needs as identified in their health-care plans.
· Maintain a balance of recreational activities including physical, social, religious, diversional, intellectual, and creative.
· Coordinate scheduled activities with other departments.
· Communicate programs to residents, resident’s families, volunteers, and facility staff.
· Identify and evaluate personal, emotional, and environmental concerns that prevent or limit the resident’s full use of medical, nursing, and restorative care.
· Formulate, review and update written plans of care for each admitted resident, according to established policies.
· Participate regularly in care conferences with nurses, and other staff to assist in better understanding each resident and his/her family.
· Responsible for transportation of residents within the community from one area to another area where activities may occur.
· Record resident’s activity participation in resident’s individual medical record and keep group records of participation on scheduled activities.
· Recruit, orientate, and supervise volunteer programs and maintain records of volunteer programs. Communicate community’s volunteer needs to the outside community.
· Prepare for meetings with Care Team by reviewing resident’s previous care plan and assessment, talking with resident to be reviewed (or a family member if resident is unable to comprehend concepts of care planning). Discuss resident’s progress and make appropriate entries in resident’s medical record. Make entries on residents’ health-care plan as required by resident’s condition.
· Participate in outside community planning related to interest of the community and arrange for the use of outside community facilities for the residents use.
· Participate in in-service program of the facility.
· Is knowledgeable to provide in-service training for staff regarding activity program, reality orientation, volunteer services, etc.
· Recognize abnormal signs and symptoms which may signify a physical change in resident’s condition that could require nursing intervention. Report same to Wellness Director immediately.
· Maintain professional confidentiality.
· May assist residents on outings which includes driving the company vehicles.

QUALIFICATIONS

· Certification: Issued by the state.
· Education: Two years of college from an accredited institution preferred.
· Experience: Minimum 6 months required, two or more years preferred.
· Job Knowledge: Awareness of special needs and dimensions of wellness for older adults; techniques and skills to engage residents with different interests and abilities. Leadership, professionalism, organization and budget skills. Ability to initiate, implement and complete projects. Familiarity with the community, social agencies, contacts with clubs and groups, and knowledge of sources of materials and local services; understanding and sensitively to residents’ needs and feelings. Computer skills adapt to new technology.
· Other: Enjoy working with older people, exercise judgment, display empathy, creative skills, imagination, friendliness, resourcefulness, enthusiasm, and have a caring attitude and sincere interest in others.
