Job Description
OnCare Home Health & Hospice is hiring a Director of Primary Care in Omaha, NE. This is a full-time position.

OnCare Primary Care doesn't. We serve residents of Heritage senior living communities across the Omaha metro, and our model is built around who they actually are: people from their 60s into their 100s who deserve a provider who knows them, visits them where they live, and follows their care over years. That model needs a dedicated leader now. Our Director of Primary Care Will: Assume full operational accountability for OnCare Primary Care, driving the strategic growth, clinical operations, community relationships, and financial health of this specialized program embedded within Heritage senior living communities, while managing the Nurse Practitioner team, reporting to the SVP of Healthcare, and maintaining direct visibility to ownership.Balance clinical rigor with sharp financial discipline as an agile operator, splitting time between the Omaha home office and our senior living communities to scale a program currently serving ~300 residents while ensuring long-term financial viability.Drive data analysis and performance improvement by owning dashboards and reporting rhythms for census, engagement, billing, productivity, and quality, extracting Electronic Health Record (EHR) and financial data to build board-ready reporting packets.Lead people through significant change without losing trust, standing up the data infrastructure to track performance, managing the billing capture engine, and partnering closely with community leaders and the NP team to enhance engagement and care quality.Optimize EHR configurations and clinical workflows, including template design, order-set optimization, workflow standardization, and system integrations, while collaborating with vendors to ensure the platform meets regulatory and practice needs.Manage financial performance and the Primary Care P& L (Profit and Loss) to meet annual targets, leading budgeting, financial modeling, and billing initiatives like coding audits, denial management, and Chronic Care Management (CCM) and Remote Patient Monitoring (RPM) expansion.Oversee clinical operations and triage by providing RN-level clinical triage support to providers, residents, and families, while ensuring total documentation integrity and compliance across Medicare Part B billing, credentialing, and RAC/TPE payer audits.Lead and develop the Nurse Practitioner team, managing their performance, productivity, and credentialing, executing our Relative Value Unit (RVU)-based or hybrid compensation model, and aligning staffing levels with patient census.Champion growth and community engagement by owning the enrollment strategy, driving conversion from palliative or supplemental services into full Primary Care, and supporting expansion into additional communities.Steer change management, communication, and culture across all clinical, operational, and technology transitions using structured frameworks like ADKAR or Kotter to align clinicians, community leadership, residents, and owners.
