In this rewarding role, you'll: Bring the Portraits Program to LifeLead our Montessori-inspired memory care program, creating personalized experiences that support each resident's physical, emotional, social, spiritual, and cognitive well-being. Get to Know Every Resident's StoryConduct resident evaluations before move-in and throughout their journey, ensuring care plans and service agreements evolve to meet their unique needs. Coach and Inspire an Amazing Care TeamTrain, mentor, be part of a team, and develop caregivers while fostering a culture of compassion, excellence, teamwork, and continuous dementia education. Champion Outstanding CareMonitor quality, conduct care observations, review documentation and response times, and implement improvements that help ensure exceptional resident experiences every day. Collaboration and Teamwork Serve as a key collaborator across the community, working closely with the Executive Director, healthcare professionals, dining services, and departmental leaders to foster a coordinated and resident-focused environment. Build Meaningful ConnectionsDevelop trusted relationships with residents, families, team members, healthcare partners, hospice providers, home health agencies, and regulatory professionals.
