In this vital role, you'll ensure that our residents receive the highest level of care while collaborating with a dedicated team, providing exceptional clinical leadership and support. What You Bring to the Table: Essential Qualifications: · Develop and maintain comprehensive care plans tailored to the needs of clients. · Coordinate services for individuals requiring senior care, home care, or support for people with disabilities. · Provide behavior management strategies and support for clients dealing with dementia or other cognitive challenges. · Assist with medication administration and ensure compliance with prescribed treatments. · Conduct regular assessments of client progress and adjust care plans as necessary. · Collaborate with healthcare providers, family members, and other stakeholders to ensure continuity of care. · Facilitate communication between clients and their families regarding care options and progress. · Maintain accurate documentation of client interactions, care plans, and any changes in condition.
