Trinity Hospice is hiring a Hospice Chaplain - PRN in Joplin, MO. This is a full-time position.

Job Summary: Primary function is to provide spiritual care in the home to terminally ill patients and their families. To provide an assessmentof the patients and familys spiritual needs and to provide counseling to meet those needs. Makes reasonable efforts tofacilitate visits by local clergy, pastoral counselors or other individuals who can support the patients spiritual needs. Participates as a core member of the Interdisciplinary Group. Job Qualifications: Education: Master of Divinity, preferred. Licensure: Per state guidelines, if any Experience: One (1) year experience as a Chaplain in a health care institution preferred. Skills: Working knowledge of community spiritual and other resources. Good interpersonal skills. Transportation: Reliable transportation. Current auto liability insurance and drivers license if personal vehicle is used for work. Assess hospice patient and support system to identify spiritual needs as evidenced by POC, documentation, clinical records, and IDG conference minutes. Provide support for the spiritual needs of the patient/family.Attend interdisciplinary team meetings and participate in the patient care planning process. Participated in Quality Assessment Performance Improvement process as required.Use effective interpersonal relations and communication skills; facilitates the use of these skills by other team members to achieve desirable outcomes.Meet mandatory continuing education requirements of the agency.Demonstrate commitment, professional growth and competency by maintaining a working knowledge of public and private eligibility standards and requirements.Promote Agency philosophy and administrative policies to ensure quality of care.Demonstrate understanding of coordination and continuity of care by collaborating with the IDG.Accept clinical assignments that are consistent with education and competence to meet the needs of the patients/families.
