Position: Skilled Nursing Memory Care Coordinator

Position Highlights:

• Supervises and evaluates work performance of nursing staff.
• Works closely with Marketing Coordinator and Social Services staff to review and assess potential move-ins
• Monitors care provided to be sure policies and procedures are followed.
• Carries out disciplinary actions as stated in personnel policies.
• Completes admission, discharge and transfer documentation as needed.
• Monitor nursing documentation for accuracy and completeness.
• Ensure daily assignments are being followed.
• Respond to all family and resident concerns.
• Participates in the Quality Improvement/Quality Assurance program.
• Completes payroll review in an accurate and timely manner
• Available "on-call" to ensure staffing coverage.
• Participates in leadership and department head meetings
