LTC - Support Service Specialist (S3)

In this role, you will be responsible for performing various administrative duties including managing files, opening and closing cases in the Compass system. Ensure all services authorizations and care plans are accurate and up to date. Document in constituents medical records daily and as needed. Responsible for processing check requests for utility, security and/or rent deposits. May request money orders for miscellaneous items needed for participant to transition. Maintain and update all tracking reports and referral logs for all Long-Term care programs. Attend department and division meetings when requested. Provide back-up support for medical records when requested. Excellent customer service, communication and interpersonal skills. Ability to work with diverse populations. Computer skills should include proficiency in Microsoft 365 Office applications (Excel, Power Point, Outlook, Word). Excellent organizational and documentation skills. Knowledge and compliance with Health Insurance Portability & Accountability Act (HIPPA) rule.
