Maximus is excited to invite a compassionate and skilled LPN to join our team as a Clinical Assessor. In this rewarding role, you will conduct person-centered assessments to evaluate Long Term Care (LTC) needs and assist in determining appropriate facility placement, all in accordance with contract directives, company processes, and program requirements.
As a Nurse Assessor, your focus will be on assessment rather than direct hands-on care. You will work with individuals in a variety of settings, including homes, facilities, and group homes, ensuring that each assessment aligns with the highest standards of care and service.
Key Responsibilities:
- Conduct thorough assessments for individuals requiring Long Term Care services
- Collaborate with other healthcare professionals to determine facility placement and care needs
- Travel within the region to complete assessments in diverse settings
About the Program:
Maximus has been selected to partner with the State of Maine's Department of Health and Human Services, through the Office of Aging and Disability Services (OADS), as the manager of its Statewide Assessing Services. This comprehensive program includes key initiatives such as Long Term Care (LTC), Acquired Brain Injury (ABI), Other Related Conditions (ORC), Intermediate Care Facilities for Individuals with Intellectual Disabilities (ICF/IDD), and Group Residential Placements (GPU).

Essential Duties and Responsibilities:
- Responsible for completing medical and/or behavioral health assessments within contract requirements
- Conducts on-site, video call and/or telephonic assessments to determine an individual's needs for services and supports, eligibility, level of care or related outcome
- Perform all job duties in compliance with Person First standards, HIPAA guidelines, and company confidentiality policies and procedures.
- Travel may be required based on program contract requirements
- Performs other related duties as assigned.
Minimum Requirements
- Current Licensed Practical Nurse (LPN) license valid in the state of practice is required
- High School Degree or equivalent required
- Minimum 2 years of clinical experience required
- LPN License in the state of Maine or hold a compact LPN license required
- Regional travel is required to perform in-person assessments
Preferred Skills- Experience in long term care or home health care

Pay Transparency
Maximus compensation is based on various factors including but not limited to job location, a candidate's education, training, experience, expected quality and quantity of work, required travel (if any), external market and internal value analysis including seniority and merit systems, as well as internal pay alignment. Annual salary is just one component of Maximus's total compensation package. Other rewards may include short- and long-term incentives as well as program-specific awards. Additionally, Maximus provides a variety of benefits to employees, including health insurance coverage, life and disability insurance, a retirement savings plan, paid holidays and paid time off. Compensation ranges may differ based on contract value but will be commensurate with job duties and relevant work experience. An applicant's salary history will not be used in determining compensation. Maximus will comply with regulatory minimum wage rates and exempt salary thresholds in all instances.
