Join PACE! As part of a dynamic Interdisciplinary Team (IDT) in an innovative model of care, you will work closely with participants and their caregivers to help them continue to live safely in their homes and communities as independently as possible. You will work in an integrated primary care and behavioral health setting to support participants in their homes and at one of our PACE centers.Participant Navigator Responsibilities
Builds trusting relationships with participants
Provides assistance with housing applications; maintains records of pending applications; provides follow-up on applications
Acts as liaison and provides housing advocacy with various housing organizations on behalf of participants
Assists with case management tasks including, but not limited to: Food Stamps/DTA applications and re-certifications, Safelink/Assurance phone assistance, scheduling RIDE interviews, Social Security income verifications, calling Social Security to troubleshoot issues
Researches and provides participants with resource information, such as food pantries, support groups, immigration assistance
Provides money management assistance and act as liaison to representative-payees
Implements supportive interventions such as check –in calls under the direction of the social worker
Performs community outreach and accompanies participants with exceptional hardship and need to appointments in special circumstances.
Consults appropriately with clinical staff
Thoroughly documents all communication with participants in the medical record in a timely manner
Demonstrates excellent customer services skills and cultural competency with participants and caregivers of diverse backgrounds
Demonstrates strong commitment to participants, resolves case management issues as expeditiously as possible to participant satisfaction. As representative of PACE and NeighborHealth, maintains a highly professional, courteous and respectful manner with participants, caregivers and colleagues

Starting hourly rate: $24.00 up to $36.00 based on experience.
