We are seeking a motivated and skilled PRN Physical Therapist to join our team in providing high-quality care for residents in a long-term care setting. The ideal candidate will evaluate patients, develop treatment plans, and deliver therapy services to improve mobility, function, and quality of life. This role also includes collaboration with the care team, supervision of Physical Therapy Assistants (PTAs), and participation in caregiver education and discharge planning.Essential Duties and Responsibilities: - Patient Evaluation and Treatment:
- Conduct patient evaluations within 24 hours (or one business day) of physician referral; document delays with clear justification.
- Develop effective treatment plans and obtain approval from the referring physician.
- Administer treatments as outlined in the physician-approved plan of care.
- Supervision and Collaboration:
- Supervise Physical Therapy Assistants (PTAs) in direct patient care and related activities, following state practice acts.
- Assist the nursing department in training Restorative Aides to support patient care.
- Communicate regularly with supervisors and healthcare team members about patient progress, challenges, and treatment plans.
- Care Coordination and Planning:
- Participate in Patient Care Conferences, Utilization Review meetings, and Rehabilitation Conferences as needed.
- Recommend and assist with ordering durable medical equipment (e.g., braces, walkers, canes, elevated toilet seats, grab bars) to promote patient independence.
- Actively engage in discharge planning to ensure smooth transitions and continuity of care.
- Documentation and Compliance:
- Maintain accurate and timely documentation, including evaluations, daily treatment notes, weekly progress reports, and recertification records, in compliance with state and payer requirements.
- Record and communicate treatment changes per facility policies and procedures.
- Patient and Caregiver Education:
- Provide education and training to patients, families, and nursing staff on maintenance programs and caregiver strategies for discharge preparation.
- Equipment Maintenance:
- Report any issues with therapy equipment to ensure it remains in good working condition.
- Training and Development:
- Participate in in-service training programs for facility staff.
Supervisory Responsibilities: - Supervise Physical Therapy Assistants (PTAs), aides, and student interns.
Qualifications: - Education:
- Bachelor's degree in Physical Therapy required; Master's or Doctorate degree preferred.
- Experience:
- Proficient in computer skills, including experience with programs like Casamba and Point Click Care.
- Skills:
- Ability to relate positively, effectively, and professionally with residents, families, staff, and interdisciplinary colleagues.
- Strong organizational and communication skills.
