Job Requirements:

• Indiana RN License
• 1 full year of long term care experience within the last 3 years
• Physical within the last 6 months that states you are "free from" or "without evidence of" communicable diseases
• Current PPD or Chest X-ray (we can do the PPD at no cost to you)
• 2 Professional References (RN's or LPN's)
• Clear Criminal History Report (we will obtain at no cost to you)
