Maximus is seeking experienced and high-performing Level of Care Lead Assessor to provide advanced support to our Indiana LCAR contract. This role combines direct assessment responsibilities with peer leadership, quality support, and collaboration with supervisors to ensure consistent, high-quality outcomes.

Lead Assessors serve as role models for clinical accuracy, timeliness, and professionalism. They act as a resource for their assigned regional teams, providing workflow guidance, monitoring assignment progress, and assisting with onboarding and mentoring new assessors.

*This is a hybrid role requiring travel to assigned areas in Indiana*

Essential Duties and Responsibilities:

- Responsible for completing medical and/or behavioral health assessments within contract requirements
- Conducts on-site, video call and/or telephonic assessments to determine an individual's needs for services and supports, eligibility, level of care or related outcome
- Perform all job duties in compliance with Person First standards, HIPAA guidelines, and company confidentiality policies and procedures.
- Travel may be required based on program contract requirements
- Performs other related duties as assigned.

Required Skills/Abilities:
- Preferred knowledge may include community support programs, long-term care assessment and level of care in medical, behavioral health or related programs
- Knowledge and understanding of medical and/or behavioral health diagnoses and prescribed medications
- Ability to collect data, define problems, establish facts, and draw valid conclusions
- Excellent written and verbal communication skills
- Excellent interpersonal and customer service skills
- Proficient in Microsoft Office Suite
- Excellent organizational skills and attention to detail
- Ability to work in a fast-paced environment
- Ability to work independently and meet timelines

- Perform PASRR Level I screens and LOC assessments in compliance with federal and state guidelines.

- Deliver consistent, high-quality work, ensuring accuracy in documentation, communication, and timelines.

- Uphold standards of reliability, clinical accuracy, and professionalism across all responsibilities.

- Serve as a first point of contact for a small regional group of LOC Assessors to answer routine questions and provide workflow support.

- Partner with the LOC Supervisor to monitor assignment lists and ensure team members remain on pace to meet expectations.

- Identify potential documentation errors or inconsistencies and informally support quality improvement efforts.

- Represent the team in internal meetings and communicate field-level barriers, training needs, and workflow feedback to supervisors.

- Assist with onboarding new assessors and provide informal peer mentorship as requested by leadership.

- Contribute to training discussions and help foster a culture of continuous improvement and knowledge sharing.

Minimum Requirements

- Education and licensure requirements are based on program contract requirements and are outlined in job posting
- High School Degree or equivalent required
- Minimum 6 years of clinical experience required

- Requires an Associate's degree in Nursing, a Bachelor's degree in a Human Services field, or a Master's degree with licensure such as LPN, RN, LCSW, or an equivalent credential

- Minimum 2 years of clinical experience

- Strong knowledge of Level of Care assessment standards and PASRR processes

- Familiarity with Behavioral health diagnoses, Psychotropic medications, Medical diagnoses

- Ability to assess individual needs and provide accurate counseling on support options

- Strong multitasking and prioritization skills

- Proficiency in Microsoft Office Suite (Word, Outlook, Excel)

- Ability to work independently and collaboratively in a fast-paced environment

- Strong written and verbal communication skills

Preferred Requirements

- Minimum of 3 years of clinical experience

- Minimum of 6 months experience working with pediatric population

- Minimum of 1 year experience working with adult population

- Proficient using Smartsheet

- Experience with AssessmentPro

- Experience with InterRAI
