Clinic Care Coordinator (Chronic Condition Disease Registry Coordinator) Job Title: Care Coordinator Department: IGA Clinics Report To: Meridian Site Administrator General Summary: Disease registry care coordination includes collecting, analyzing, and maintaining detailed medical data on patients with a specific disease, like cirrhosis, by reviewing patient records, tracking their care plan compliance over time to contribute to improved patient outcomes and quality improvement initiatives. Their primary responsibilities include identifying patients, extracting relevant information from medical records, ensuring data accuracy and coordinating care with the patient to ensure timely completion of care plan milestone events such as labs, diagnostic testing, or follow up appointments. The coordinator will validate the integrity of patient data while ensuring privacy, security, and confidentiality of patients' health information. This position works in coordination with the patients primary direct care MA or the Clinic RN. Care Coordination Essential Job Functions 1. Maintain internal disease registries for patient compliance to care plan for the following diseases: cirrhosis, IBD/IBS, and Ulcerative Colitis. 2. Identifying patients who meet the registry's inclusion criteria and ensuring they are included in the registry. 3. Review patient medical records to identify and extract relevant information regarding diagnosis, treatment, and follow-up care for the specific disease. 4. Input and maintain patient data in a dedicated disease registry tracking system, ensuring accuracy and completeness. 5. Communicate with patients for testing, diagnostic and follow-up appointment reminders in a timely manner. 6. Communicate with physicians and other healthcare professionals to clarify medical information and obtain necessary data for accurate registration. 7. Collaborate with Site Administrators and Quality Team to ensure data systems and programs satisfactorily support CQI activities, including maintaining QI tracking systems for patient registries and compliance. 8. Adhere to local, state, and federal regulations regarding data privacy and reporting requirements for disease registries. 9. Support practices to keep care records up-to-date by identifying and updating missing or out-of-date information about the persons circumstances; 10. Run periodic reports to ensure all patients with the disease registry diagnosis have been captured and are monitored in the internal disease registry database. Reviews quarterly QI data and prepares annual QI report to be presented to all Site Administrators relating to chronic disease registries. 11. Presents metric reports monthly and quarterly to Site Administrators. 12. Track and document in the patients chart the teams time spent in care coordination each month then submit billing for chronic care management codes. 13. Willingness to support other areas of operations assigned. Education: Minimum of clinical certified medical assistant and high school diploma. Preferred bachelor's degree in health administration, nursing, science, OR equivalent experience and education required. Experience: Minimum five years of experience in health care setting with preferred care coordination or QI experience. Other Requirements: Current CMA, MA, LPN, RN or another clinical license as required. Current CPR certificate required. Performance Requirements: Knowledge: 1. Knowledge of disease registries and data management principles. 2. Maintains awareness of current regulatory requirements, keeping abreast of pending and or implemented changes at both the federal and state levels. 3. Develop communication tools to alert the clinic teams of necessary information. 4. Ensures that required state/federal reporting is completed in a timely manner. 5. Maintains the strictest confidentiality in the areas of patient, employee and physician relations. Skills: 1. Skill in effective application of CQI teams, quality measures, clinical guidelines, and process management initiatives. 2. Skill in using computer capabilities effectively to produce needed trend and evaluation data and documentation. 3. Strong knowledge of medical terminology and disease processes 4. Expertise in coding systems like ICD and relevant disease-specific coding standards 5. Attention to detail and ability to maintain data accuracy 6. Excellent analytical and reporting skills 7. Proficiency with computer applications and database management systems 8. Strong communication and collaboration skills to work with healthcare providers Abilities: 1. Ability to work effectively through medical practice by establishing effective working relationships with all departments and levels of staff. 2. Ability to work independently and as part of a team. 3. Ability to present CQI concepts, methods, and tools in a clear and persuasive manner in verbal, written, automated, and audiovisual formats to a variety of audiences. 4. Ability to promote the importance of QI concepts related to the medical practices mission and goals. 5. Ability to maintain timely communications with patients regarding patient care plan compliance. Equipment Operated: Standard office equipment including computer hardware/software and training/presentation equipment such as audiovisual machinery. Work Environment: Office environment and occasional classroom. Well lighted and well ventilated. Rare contact with patients. Mental/Physical Requirements: Combination of sitting and standing. Occasional lifting/carrying of audiovisual equipment. Some stress related to high level of responsibility for quality care.
